The current CAP Y-Survey uses “processed” human plasma which has been manipulated significantly and has many additives for establishing appropriate concentration of many other measurands (analytes). This may lead to some degree of non-commutability of the materials with many of the clinical immunoassays.
The CAP now plans to send out fresh frozen human serum pools in upcoming Y-Survey challenges to evaluate the current test accuracy of C-peptide assays in the field. This will provide a basis of comparison before and after manufacturers recalibrate their assays.
Beginning in 2019 the CAP began an accuracy based survey for glucose, insulin and c-peptide. This is the ABGIC survey. In this survey, fresh frozen human serum pools with different c-peptide concentrations are sent to laboratories. This new survey will allow us to monitor the process of standardization before and after manufacturers recalibrate their assays.
